Responsibilities:
Provide one-on-one care and companionship to clients in their homes, including light housekeeping, meal preparation, and personal care services such as bathing and dressing.
Assist clients with daily living activities, such as mobility, transfer, and ambulation.
Monitor and report changes in clients' condition to the care team and other healthcare professionals.
Maintain accurate and detailed records of client care and services provided.
Collaborate with other healthcare professionals, including occupational therapists, physical therapists, and speech therapists, to ensure comprehensive care.
Requirements:
Certified Home Health Aide (CHHA) certification or equivalent experience.
High school diploma or equivalent required.
Ability to lift, transfer, and move clients safely and comfortably.
Strong communication and interpersonal skills.
Ability to work effectively in a fast-paced environment and adapt to changing situations.
Reliable transportation and a valid driver's license.
