Care Coordinator Indianapolis West

Are you compassionate, organized, and sales-minded? We’re looking for a motivated Care Coordinator to join our team in West Indianapolis.

In this role, you’ll:

• Answer inquiries and convert them into new clients
• Create personalized care plans for seniors
• Build strong relationships with clients, families and caregivers.
• Conduct quality assurance visits
• Conduct client assessments.
• Create social media content and post it weekly.
• Support onboarding new caregivers.
  
  
  

What We’re Looking For:

• High school diploma or equivalent
• 2–4 years of related experience (preferred similar experience candidates)
• Strong sales and communication skills
• Valid driver’s license and reliable transportation
• Comfortable working some evenings/weekends
