Job Summary
We are seeking a proactive and compassionate Bilingual Patient Access Outreach Coordinator to join our healthcare team. The primary function of the position is to answer and respond to all inquiries from patients scheduled for appointments. The Patient Access Outreach Coordinator assists patients with securing their specialty medical care appointment. They help the Program Manager as needed and participate as an essential team member in building Access Now’s capacity to meet the growing need for specialty care access in the Richmond community.
Duties
- Fluent in Spanish and English
- Initiates and responds to inquiries from patients requesting program information and assistance Communicate with patients via Caremessage for text-based interactions.
- Monitors, responds to, and returns patient voicemail messages in a timely manner
- Conducts outbound calls to patients from assigned call lists to assist with closing out referrals and ensuring proper follow-up
- Pulls and reviews reports of expired or expiring referrals and enrollments and notifies appropriate clinics to ensure continuity of care
- Responds to patient concerns and refers issues to the Program Manager as necessary
- Documents/charts all communication with patients
- Communicates with patients for specialists’ offices to obtain services and coordinate care
- Assists patients in need of Medical Interpreter Volunteers
- Accepts inquiries from patients requesting specialty appointments submitted by clinics as needed Informs patient of rights and responsibilities.
- As part of the care coordination team, maintains all forms and manuals related to the program and is responsible for the timely distribution of updated information to partners
- Collaborates on the content, design, and distribution of communications materials
- Makes recommendations for improvements to the program services and documents
- Interacts professionally on a regular basis with staff, clinics, physicians and the general public
- Attends and helps facilitate clinic meetings and trainings.

- Familiarity with health regulation policies related to patient privacy, confidentiality, and data security standards (e.g., HIPAA compliance)
- Skilled in using patient management software and electronic health records (EHR/EHR) systems for data entry and retrieval
- Excellent customer service skills with the ability to communicate clearly and empathetically over the phone and in person
- Strong organizational skills with attention to detail; proficient typing skills for accurate data entry
- Ability to handle sensitive information discreetly while managing multiple tasks efficiently
