We hope you'll join our team!The OpportunityRhode Island Primary Care Physicians Corporation (RIPCPC) is seeking a Patient Navigator to support patients across its 65 primary care practices located throughout Rhode Island. This role focuses on supporting practices and their patients through post-visit and behavioral health interventions. The Patient Navigator complements the activities of embedded office staff and other wraparound team members employed centrally at RIPCPC. This position plays a key role in coordinating care, facilitating referrals, and improving patient access and continuity of care.What you'll doAct upon referrals ordered during office visits and facilitate appointment scheduling and referral entry in insurance-based systems, prioritizing BCBS and UHC plansEnsure accurate and thorough documentation of referrals within the EMR at every step of the referral process from initiation to closure, including closing the loopEnsure documentation is collected from patient specialist visitsIdentify if SDOH needs are present through coordination of services and handoff to care team members where appropriateSchedule appointments both internally and externally with outside specialistsFacilitate referrals between care team membersAct upon behavioral health referrals ordered during office visits and facilitate appointment schedulingCollaborate with RIPCPC clinicians for timely behavioral health referrals and schedule appointments via the RIPCPC booking siteFacilitate relationships with community organizationsField calls for patients seeking a new primary care physician and provide contact information for RIPCPC providers accepting new patientsDirectly schedule new patients via Epic for select officesFacilitate Uber Health rides to patients' primary care offices via the Uber Health PortalAttend and participate in related meetings and perform other job-related duties as assignedWho you are2–3 years in the medical field, preferably with a Medical Assistant or Quality Assistant background (preferred)Experience with patient-facing roles, particularly one involving telephonic outreach (preferred)Experience in primary care practices (strongly preferred)Experience with value-based metrics around quality and cost containment (preferred)Experience with EHRs, particularly Epic, eClinicalWorks, and AthenaComfortable with changing day-to-day priorities and assignmentsComfortable working in a teamExcellent verbal communication and interpersonal skillsDetail-oriented, self-motivated, and comfortable in ambiguityProficiency in using the Microsoft Office SuiteBachelors degree in healthcare related field preferedBenefitsPaid time off: 18 days per year, plus 11 paid holidaysMedical, Dental, and Vision insuranceHealth Reimbursement Arrangement to reduce deductible liabilityOptional Health Savings Account (HSA)Prescription drug coverageCompany-paid life insurance with option for additional voluntary life insuranceLong-term disability401(k) eligibility after 1 year of employment (employer contributions immediately vested once eligible and participating)This is an hourly postion with the range: $20-23/hourSalary range$41,600—$47,840 USDRhode Island Primary Care Physician's Corporation (RIPCPC) is an equal opportunity employer, and we encourage qualified applicants of every background, ability, and life experience to contact us about appropriate employment opportunities.
