Bookmark Medical is seeking an experienced Internal Medicine/Adult Medicine Advanced Practitioner (NP or PA) to join our established Primary Care team in Brockton, MA. This well-respected community practice includes two Physicians, serving patients across all ages in a supportive, patient-first environment.
$120,000 - $145,000 Base salary incentives

Responsibilities
- Provide primary care to an existing adult and geriatric patient panel
- Diagnose and manage acute and chronic conditions
- Conduct Annual Wellness Visits and develop personalized treatment plans
- Order and interpret diagnostic tests
- Collaborate with specialists and leverage technology to deliver coordinated care
Qualifications
- Graduate of an accredited NP or PA program; current certification and licensure
- At least one year of APP experience, preferably in primary care
- Strong clinical skills, collaborative mindset, and proficiency with EMR systems
