And you want to be part of a collaborative team that helps families stabilize, navigate the housing system and achieve meaningful goals.Position SummaryThe Case Manager provides intensive, strengths-based case management services to families who are frequent users of the shelter system. This role supports families in accessing stable housing, benefits and community resources while collaborating closely with shelter partners to prevent discharge and ensure ongoing engagement. The Case Manager reports to the Clinical Case Manager Lead for the FUSE Program.Position ResponsibilitiesProgram Delivery (70 percent)Provide comprehensive case management services to assigned households to support stabilization in housing, maintenance or increase of income, and increased skills and resourcesComplete intake, assessments, service plans, CES referrals and required follow-upAssist families in accessing benefits including SNAP, TANF, SSI, childcare and transportation supportProvide life skills instruction in formal and informal settings including financial literacy, tenant education, parenting education and health and wellness accessMaintain regular contact with families in shelters and community locations to support consistent engagementUse motivational interviewing and harm reduction strategies to support progress toward stability, income growth, improved health and family routinesFacilitate coordination with school enrollment and STLS services as neededCollaborate with shelter providers to prevent discharge and ensure continuity of servicesAdvocate on behalf of families with housing system partners and community resourcesProvide crisis intervention, de-escalation and safety planning when neededSupport linkage and warm handoffs to medical, mental health, legal and employment servicesConduct follow-up with households after housing placement in accordance with program guidelinesParticipate in interdisciplinary case staffing with clinical and OT/COTA providersProgram Administration (30 percent)Document all interactions, referrals and services in HMIS and agency databases within required timeframesMaintain accurate, organized and confidential electronic and paper recordsComplete all required documentation including assessments, service plans, compliance forms and closing proceduresContribute to performance reporting and quality improvement effortsAdhere to agency policies, ethical standards and funding compliance expectationsQualificationsBachelor’s degree in social work, psychology or related human services field preferredOne year of case management experience preferredStrong interpersonal and communication skills, including crisis response and rapport buildingAbility to work effectively with families who may experience trauma, disability, mental health needs, substance use or system involvementKnowledge of the homeless service system in Chicago preferredProficiency with Google Suite and electronic data systemsBilingual Spanish and English strongly preferredCommitment to the mission of ReVive Center and to trauma-informed, strengths-based careCompensation and BenefitsUp to $50,000 annual salary, commensurate with experienceFull benefits including medical, dental, vision and life insurance403(b) with employer contributionPaid Time Off per policyWorking Conditions and Physical RequirementsTravel required between shelters, engagement center and community locationsWork is performed in shelters, offices and client residencesIndependent home visits requiredAbility to sit for extended periods as well as intermittent physical activity including standing, walking, stair climbing and lifting up to 10 poundsEffective verbal communication and functional hearing, vision and manual dexterity or reasonable accommodationsEqual Opportunity Statement:ReVive Center is an equal opportunity employer.
