A confidential home health agency is seeking a compassionate Home Health Registered Nurse (RN) to provide skilled nursing care to patients in their homes. Key Responsibilities
Perform patient assessments and collaborate with the care team to create and implement individualized plans of care.
Promote patient health and independence through teaching and rehabilitative measures, including self-care education.
Make referrals to other disciplines as needed and document clinical decisions accurately.
Supervise and provide direction to LPNs and Home Health Aides.
Complete all documentation accurately and in a timely manner.
Communicate patient progress and care updates with clinical managers and team members.
Participate in staff education, continuing education programs, and clinical development initiatives.
Provide on-call support and services to patients and families as assigned.
Qualifications
Current RN license in the state of South Carolina.
MediClear certification or equivalent required.
Valid driver’s license, reliable transportation, and liability insurance.
Minimum of 1 year of clinical RN experience preferred
