Role Description
The MDS Coordinator is a full-time, on-site position located in Morrison, IL. The role involves managing the coordination and completion of the Minimum Data Set (MDS) assessments and care plans to ensure compliance with regulations and optimal resident care. Responsibilities include collaborating with the interdisciplinary team, assessing resident needs, developing individualized care plans, and ensuring accurate documentation. The MDS Coordinator will work closely with nursing staff to enhance the quality of care for residents in alignment with organizational values and goals.
Qualifications
- Active Licensed Practical Nurse (LPN) or Registered Nurse (RN) license
- Strong expertise in MDS processes and care planning
- Experience with care plans and geriatric nursing
