Position Summary

The Registered Nurse (RN) – Telephone Triage is responsible for providing clinical support to patients by telephone while ensuring safe, timely, and compassionate care. This position assesses patient concerns, provides appropriate nursing advice within the RN scope of practice, communicates with providers regarding patient needs, coordinates care, and documents all patient interactions accurately in the electronic medical record (EMR). The RN plays a vital role in maintaining patient satisfaction, promoting positive health outcomes, and ensuring efficient communication between patients, providers, pharmacies, and healthcare facilities.

Essential Duties and Responsibilities

Patient Care

· Answer incoming patient telephone calls in a professional, courteous, and timely manner.

· Perform clinical telephone triage using established nursing protocols and critical thinking skills.

· Assess patient symptoms and determine the appropriate level of care, including emergency intervention when necessary.

· Provide patient education regarding disease processes, medications, treatments, and provider instructions.

· Reinforce provider treatment plans and follow-up recommendations.

· Recognize urgent or emergent situations and direct patients to the appropriate level of care, including calling 911 or proceeding to the Emergency Department when indicated.

· Assist in establishing protocols

Clinical Coordination

· Review and prioritize patient messages throughout the day.

· Communicate patient concerns and clinical findings to providers in a concise and organized manner.

· Review laboratory and imaging results as instructed by providers and communicate results to patients.

· Process medication refill requests according to office policy and provider protocols.

· Obtain prior authorizations or coordinate with appropriate staff when needed.

Documentation

· Document all patient interactions accurately, thoroughly, and promptly in the electronic medical record.

· Ensure documentation includes patient assessment, nursing interventions, provider communication, patient education, and follow-up plans.

Collaboration

· Work closely with physicians, nurse practitioners, medical assistants, scheduling staff, and other departments to ensure continuity of patient care.

· Assist with care coordination for complex or high-risk patients.

· Participate in daily team communication regarding patient needs and workflow.

Quality & Compliance

· Maintain patient confidentiality in accordance with HIPAA regulations.

· Follow all OSHA, infection control, and practice safety guidelines.

· Adhere to nursing standards of practice, state nursing regulations, and organizational policies.

· Participate in quality improvement initiatives and ongoing staff education.

Qualifications

Education

· Graduate of an accredited Registered Nursing program.

· Current Registered Nurse (RN) license in the state of practice.

Experience

· Minimum of two years of clinical nursing experience preferred.

· Telephone triage experience preferred.

· Experience in pulmonary, internal medicine, family medicine, or other outpatient specialty practice preferred.

· Experience with electronic medical records (EMR/EHR) required.

Knowledge, Skills, and Abilities

· Excellent assessment and critical thinking skills.

· Strong knowledge of outpatient nursing principles and telephone triage protocols.

· Ability to prioritize multiple tasks in a fast-paced environment.

· Excellent verbal and written communication skills.

· Strong organizational and time management skills.

· Ability to maintain professionalism and empathy when interacting with patients.

· Ability to work independently while collaborating effectively with the healthcare team.

Physical Requirements

· Prolonged periods of sitting while working at a computer.

· Frequent telephone communication.

· Ability to view computer screens for extended periods.

Work Environment

This position is performed in a medical office setting and requires frequent interaction with patients, providers, pharmacies, hospitals, laboratories, and other healthcare professionals. The RN must be able to effectively manage interruptions, prioritize urgent patient needs, and maintain productivity throughout the workday.

Pay: $27.00 - $33.00 per hour
