Job Summary
Provides assistance with basic needs, in-home visits, transportation, and maintaining all required documentation.
Responsibilities
· Handles all case management referrals.
· Assists clients with obtaining basic needs.
· Contributes to overall fundraising as well as health fairs etc.
· Enroll clients in eligible programs (food stamps, Medicaid, PHC, etc)
· Translation services for non-English speaking clients (if Bilingual)
- Work with Outreach & Enrollment Specialist to assist patients (e.g. Medicaid application).
- Adhere to HIPAA guidelines.
- Maintain Certifications and required annual trainings
- Performs other duties as assigned by supervisor.
Minimum Requirements
- Computer; calculator; time management; computer spreadsheet skills.
- Experience using Microsoft Word and Excel, strong numerical aptitude.
- Initiative; flexibility; ability to effectively address multiple tasks requiring attention to detail; problem identification, analysis, and resolution.
- Ability to work under pressure to meet deadlines; ability to function with autonomy and minimal supervision.
· Ability to manage confidential records and information.
- Excellent communication and problem solving skills with strong attention to detail.
Educational Qualifications
· Must have a high school diploma or equivalent (required).
· Two years experienced in social services (preferred).
Typical Physical Demands
· Finger Dexterity: Using fingers to make small movements such as typing or picking up small objects. Normal fine and gross motor control of fingers and hands.
· Talking: Frequently conveying detailed or important instructions or ideas accurately, clearly, or quickly.
· Hearing: Able to hear average or normal conversations and receive ordinary information; hear whispered voice at five (5) feet.
· Repetitive Motions: Frequently and regularly using the wrists, hands, and fingers.
· Visual: Visual acuity necessary for the proper evaluation of a patient and to prepare or inspect documents or other materials. Ability to accurately view computer monitors
· Physical: Sitting 70-80% of time and standing, walking 20-30% of time. Lift up to 40 lbs; push and pull up to 40 lbs. Able to bend at the waist, twist and turn along axial plane.
