Job Summary
The On-Call Hospice Registered Nurse (RN) is responsible for organizing and directing hospice care with a strong foundation in nursing and community health education. This role involves responding to after-hours patient needs, including admissions and providing immediate care in patients' homes and communities. You will work closely with the hospice team to ensure that care plans are followed and adjusted as needed, helping patients achieve maximum comfort.

Core Responsibilities
- Available to receive and respond to calls from Triage during assigned shifts
- Conduct visits to patients with changing conditions and complete weekend admissions
- Complete comprehensive assessments for patients and families during after-hours calls
- Provide skilled nursing care, including administering medications and treatments as prescribed
- Write, initiate, and revise care plans based on patient status and needs
- Counsel patients and families in managing nursing needs and self-care
- Participate in interdisciplinary group meetings and contribute to the ongoing care coordination
- Maintain accurate and timely clinical notes and communicate changes to the physician
- Remain available for all patient care calls during on-call hours, including crisis management
- Assist in ensuring that necessary equipment and services are arranged
Qualifications
- We're excited to train RN's that are passionate about hospice care
- RN license
- 1-2 years recent acute care experience (preferred)
- 1-2 years home care experience (preferred)
- Valid driver's license and reliable vehicle
- Strong communication and interpersonal skills
- Empathy and comfort with end-of-life care
Work Environment
- Mix of facility and home-based care
- 7 days on and 7 days off (Monday night – Monday morning)
- Centralized territory assignment
