Resolve Harmony Center is seeking an experienced and detail-oriented Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team as an MDS Coordinator. This position plays a vital role in ensuring accurate assessments, regulatory compliance, quality outcomes, and appropriate reimbursement within our skilled nursing facility.
Responsibilities:
- Complete and coordinate MDS assessments in accordance with state and federal regulations.
- Ensure accuracy and timeliness of MDS submissions and supporting documentation.
- Coordinate the interdisciplinary care planning process.
- Review clinical documentation to ensure appropriate reimbursement and regulatory compliance.
- Monitor quality measures and identify opportunities for improvement.
- Collaborate with nursing leadership, therapy, social services, and other departments.
- Assist with Medicare and Medicaid case management related to resident assessments.
- Maintain knowledge of current regulations and reimbursement guidelines.
- Support survey readiness and compliance initiatives.
Qualifications:
- Current RN or LPN license in good standing.
- Must have a valid AZ Fingerprint Clearance Card
- Previous MDS Coordinator experience in a skilled nursing facility preferred.
- Strong understanding of Medicare, Medicaid, PDPM, and MDS regulations.
- Knowledge of care planning and reimbursement processes.
- Excellent organizational, communication, and time management skills.
- Ability to work independently while collaborating with an interdisciplinary team.
- Strong attention to detail and ability to meet deadlines.
