Summary
Job Summary:
The Physical Therapist is responsible for managing and delivering skilled physical therapy services to patients with a wide variety of diagnoses and neurological conditions. This role involves evaluating patient needs, developing and implementing individualized treatment plans, and providing professional consultation to help patients achieve maximum independence based on their functional level and potential. Care is delivered with consideration for each patient’s physical condition, age, and specific needs.
Responsibilities
Key Responsibilities:
Perform comprehensive patient evaluations and develop individualized treatment plans
Deliver evidence-based physical therapy interventions for outpatient neurological conditions
Monitor patient progress and adjust treatment plans as needed
Provide education to patients and caregivers on therapeutic exercises and recovery strategies
Collaborate with interdisciplinary teams to optimize patient outcomes
Maintain accurate and timely documentation in compliance with regulatory standards
Ensure a safe and supportive clinical environment
Qualifications
Required Qualifications:
Bachelor’s, Master’s, or Doctor of Physical Therapy (D.P.T.) degree from an accredited Physical Therapy Program
Oregon Physical Therapist License upon hire
National Provider BLS certification from the American Heart Association within 30 days of hire
Ability to meet department-specific training requirements or possess equivalent education and experience
May require specialized physical therapy training or experience depending on department needs
