Position Overview:

The Lead Mental Health Worker oversees and coordinates services for individuals receiving support in the community. This role is responsible for guiding clients to achieve independence in their daily lives and for supervising and mentoring Community Mental Health Workers (CMHWs). The Lead Community Mental Health Worker ensures compliance with client-specific goal plans, statutory requirements, and program policies. This position also oversees the 24-Hour Emergency Assistance program, which includes emergency interventions, crisis prevention, and client support. The role requires strong leadership in crisis situations, administrative duties, and collaboration with both internal and external stakeholders to enhance client outcomes. The Lead Community Mental Health Worker will provide mentorship to CMHWs and act as a resource for staff in the program.
Role Responsibilities:
  • 24-Hour Emergency Assistance (EA):
    • Responsible for the 24-hour emergency program Monday – Friday during business hours
    • Respond promptly to emergency calls, providing on-call counseling and immediate crisis intervention at clients' homes or other locations.
    • Perform emergency visits to clients’ homes and other community locations as needed.
    • Maintain detailed documentation of emergency calls, visits, and actions taken.
    • Act as the key point of contact for staff involved in the 24-Hour Emergency Response Program.
    • Conduct regular check-ins with clients, updating Emergency Plans and related documents as needed.
    • Provide ongoing emotional support and assist clients in practicing coping mechanisms.
    • Participate in the on-call rotation and may cover additional shifts as needed.
  • Supervision & Team Leadership:
    • Mentor and provide guidance to CMHWs to ensure high-quality service delivery.
    • Assist the Program Manager in supervision and team development, including sitting in on supervision sessions as needed.
    • Facilitate regular team meetings, discuss challenges, and create strategies to improve client outcomes.
    • Provide direct guidance and support during crisis situations, acting as the escalation point for complex issues.
    • Mentor CMHWs in the application of crisis management techniques, including de-escalation and non-violent intervention strategies.
  • Client Care & Support:
    • Provide support on the implementation of IHS plans for clients.
    • Ensure clients' Goal Plans, CSSP, and IAPP are aligned and adhered to, adjusting as necessary.
    • Monitor clients' progress toward goals, making appropriate updates to care plans.
    • Facilitate regular client meetings and ensure progress notes and documentation are accurately recorded.
    • Provide direct support to clients, teaching and modeling independent living skills (ILS), including health, wellness, community participation, and adaptive skills.
    • Assist clients in problem-solving and provide supportive crisis counseling to prevent or manage behavioral crises.
    • Coach clients on coping skills and strategies outlined in their person-centered plans.
  • Program Oversight & Compliance:
    • Ensure compliance with all program policies, statutory regulations, and the 245D statute.
    • Manage a caseload of IHS clients, ensuring all goals are tracked, services delivered, and visits conducted in a timely manner.
    • Communicate regularly with clients, families, and stakeholders about client progress, goals, and service needs.
    • Monitor the 24-Hour Emergency Response Program to ensure adherence to documentation standards, client assessments, and overall service delivery requirements.
    • Maintain accurate records for client services, tracking incidents, goals, and updates in RTI's EMR system (Therap).
  • Administrative & Documentation:
    • Review and approve weekly progress reports and incident documentation submitted by CMHWs.
    • Ensure all client documentation, including goal plans, emergency plans, and annual reviews, is up to date.
    • Perform administrative tasks, such as charting, office cleaning, and coordinating updates to client records.
    • Ensure documentation adheres to both internal policies and external regulatory guidelines.
  • Collaboration & Communication:
    • Serve as the primary contact for clients, families, and external providers, ensuring effective communication.
    • Foster collaboration with internal staff, external providers, and other stakeholders to improve client care and outcomes.
    • Escalate concerns regarding client abuse, neglect, or financial exploitation according to protocol.
    • Facilitate ongoing communication between program staff and key internal personnel, such as Program Managers and Directors.
  • Professional Development & Staff Training:
    • Provide ongoing training and professional development opportunities for CMHWs, including new staff orientation and in-service training.
    • Participate in staff meetings, supervision, and training sessions to maintain certification/licensure and enhance professional knowledge.
    • Assist with the onboarding and training of new team members, ensuring they are equipped to perform their duties.
    • Stay current with trends and best practices in mental health care, particularly in crisis management and community-based services.
  • Other Duties:
    • Perform additional duties as assigned by the Program Manager, Program Supervisor, or Program Director.
    • Participate in the 24-hour emergency on-call rotation as required, offering support to staff and clients during after-hours crises.
Required Qualifications and Experience:
  • Associate degree in Behavioral Sciences, Social Work, or a related field from an accredited institution.
  • Experience with mental health care and community services, especially within the context of independent living and client goal planning.
  • Certification in First Aid and CPR (or willing to obtain).
  • Must possess a bachelor’s degree in social work, psychology, or a closely related human services field; or a minimum of three (3) years of experience working in mental health and/or intake processes, including, but not limited to: mental health, community outreach, marketing, and stakeholder communication. An equivalent combination of education and experience may be considered.
  • Must pass the required background check and remain eligible to provide services as defined by the Minnesota Department of Human Services.
  • Must pass the screening required by the Federal Human Services Office of Inspector General to ensure the person has not been excluded from working in Health Care Programs which receive federal funding.
  • Must have access to a reliable vehicle and maintain a valid driver’s license and insurance.
Preferred Qualifications:
  • Bachelor’s degree in one of the behavioral sciences or related fields from an accredited college or university.
  • 2000 hours of experience in delivering clinical services in the treatment of mental illness or children with emotional disturbances.

Compensation: $19- $21 per hour

  • Walk, stand, sit, bend, climb stairs, and perform crisis prevention/intervention techniques
  • Use hands for writing, typing, and data entry

  • Maintain vision and hearing necessary for reading charts, operating office equipment, using a computer, and driving a vehicle
