Responsibilities

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting, ensuring comfort and dignity.
• Monitor patient health by observing vital signs such as blood pressure, pulse, temperature, and respiration; report any abnormalities promptly.
• Support medication administration following medical instructions and maintaining accurate medical documentation in Electronic Health Record (EHR) systems.
• Provide specialized care for clients with dementia, Alzheimer’s disease, or other memory impairments, including memory care techniques and Alzheimer’s care strategies.
• Assist with mobility and transfers using patient transfer methods such as Hoyer lifts or other equipment to ensure safety during movement.
• Perform light housekeeping duties such as cleaning, laundry, meal preparation, and maintaining a safe environment in compliance with home healthcare regulations.
• Document all patient interactions accurately while maintaining strict confidentiality in accordance with HIPAA standards.
• Support end-of-life or hospice care needs with compassion and professionalism, respecting client wishes and family involvement.
• Use effective communication skills to coordinate with healthcare teams and family members to ensure comprehensive patient care.
• Follow safety protocols including first aid, CPR certification requirements, and basic life support procedures to respond effectively during emergencies.

Experience

• Prior experience working with seniors or geriatric patients in home healthcare settings or assisted living facilities preferred.
• Demonstrated knowledge of medical terminology, catheter care, wound management, and medication administration.
• Experience working with individuals with developmental disabilities or disabilities in general enhances caregiving effectiveness.
• Familiarity with EMR (Electronic Medical Records) systems and home health agency procedures is highly desirable.
• Proven ability to perform heavy lifting safely and operate mobility aids such as Hoyer lifts or patient transfer devices.
• Knowledge of home & community care practices along with long-term care experience is advantageous.
• Certification in first aid, CPR, and basic life support is required or preferred to ensure readiness for emergency situations.
