Job Title: LPN / LVN - Long term Care

Location: Utica, NY 13502

Duration: 13 Weeks contract (Possible Extension)

Shift: 10:30pm - 6:30 am

Responsibilities:

  • Provide direct patient care to residents with conditions such as MS, orthopaedic injuries, dementia, and memory care needs.
  • Administer medications and treatments in accordance with facility policies and procedures
  • Monitor, record, and report changes in patient conditions.
  • Collaborate with the healthcare team to develop and implement individualised care plans.

Requirements:

  • At least 1 year of experience in LTC facility.
  • Active LPN license for the state on New York.
