Home Health RN Responsibilities

Provide skilled nursing care in the patient’s home setting, including assessments, treatments, wound care, and medication management.

Develop and implement individualized home health care plans, adjusting as patients’ conditions change.

Educate patients and families on health conditions, treatments, and at-home care.

Collaborate with the Director of Nursing, physicians, and interdisciplinary teams to coordinate quality care.
Qualifications for Home Health RN Jobs

Graduate of an accredited nursing program (BSN preferred).

Current Arizona Registered Nurse (RN) license in good standing.

At least 1 year of professional nursing experience (home health nursing experience strongly preferred).

Public Health Nurse (PHN) certification a plus.

Strong clinical judgment, communication, and organizational skills.

Valid driver’s license, reliable transportation, and ability to travel to patient homes in Sun City and surrounding Maricopa areas.
