Your Role: Home Health LVN
As a Licensed Vocational Nurse (LVN) in Home Health, you will deliver skilled nursing care to patients in their homes under the supervision of a Registered Nurse (RN). You’ll play a vital role in supporting patients through rehabilitation, chronic condition management, and senior care services.
Key Responsibilities
- Provide skilled home health nursing care based on individualized care plans
- Support senior care and rehab patients in home and community settings
- Document patient visits, treatments, and progress in EMR systems
- Collaborate with physicians, RNs, families, and interdisciplinary teams
- Assist patients with chronic conditions, recovery, and dementia care
- Ensure compliance with California home health regulations
- Promote patient safety, independence, and overall well-being
Qualifications
- Active California Licensed Vocational Nurse (LVN) license in good standing
- Graduate of an accredited vocational nursing program
- Minimum 1 year of nursing experience (home health preferred)
- Experience in senior care, rehab, or community care preferred
- Valid driver’s license, reliable transportation, and auto insurance
- Strong communication, organization, and documentation skills
- Experience with electronic medical records (EMR systems)
