A recovery and wellness center near the Lakeland, FL area is currently searching for a full time and permanent Case Manager to join team.

Responsibilities of the Case Manager - Substance Use Treatment:
- Care coordination, including intake screenings and navigation of community resources
- Treatment & discharge planning while collaborating with clinical team
- Provide Group Facilitation
- Complete documentation in a timely manner

Requirements of the Case Manager - Substance Use Treatment:
- Case Manager experience within the substance use treatment field required
- Active CBHCM, CAP or CAC certification
