Position: Social Worker – Clinical (LCSW)
Location: 900 23rd Street NW Washington DC USA 20037
Shift: 8 Hour Day Shift (8:30 AM – 5:00 PM)
Duration: 13 Weeks
Job Summary:
We are seeking an experienced Clinical Social Worker (LCSW) – Case Management to join the care team at The George Washington University Hospital. The Social Worker will be responsible for discharge planning, care coordination, and providing psychosocial support to patients and families to ensure safe and timely transitions of care.
Required Qualifications:
- Education: Master’s degree in social work (MSW) required.
- Experience: Minimum 1 year of hospital case management experience required.
- Strong experience in discharge planning required.
- Licensure & Certifications: Active LICSW License required.
- Active Washington, DC Social Worker License required.
- Technical Skills: Experience with Cerner EMR required.
Additional Requirements:
- Must reside 50 miles or more from the facility (Travel requirement).
- Strong communication and coordination skills.
- Ability to work effectively in a fast-paced hospital environment.
- Knowledge of community resources and post-acute care services preferred.
Salary : $45 - $50
