Job Description

Supervisor: Clinical Manager

Job Purpose:

A Registered Nurse provides nursing care/services in accordance with the patient’s plan of care as directed by their primary care provider. The Registered Nurse supervises Licensed Practical Nurses (LPNs) and Home Health Aides per Medicare, Medicaid regulations, as well as state regulations, if any.

Qualifications:

1. A graduate of an approved school of professional nursing who is licensed as a Registered Nurse by the state in which practicing.

2. At least one year of nursing experience preferred.

3. Home health care experience preferred.

Training:

Outcomes and Assessment Information Set (OASIS) training preferred.

Responsibilities:

1. Provides comprehensive assessments (initial and ongoing) including OASIS items for Medicare and Medicaid patients.

2. Develops and implements the patient’s plan of care in accordance with the patient’s primary care provider.

3. Reviews the patient’s medications and updates the patient’s medication profile when there are new, changed, or discontinued medications.

4. Evaluates the patient or caregiver/family member’s ability to manage medications appropriately.

5. Assigns and supervises LPNs and Home Health Aides as required by Medicare/Medicaid and state regulations, if any.

6. Participates in orientation and training of new personnel.

7. Evaluates and completes competencies for Registered Nurses, LPNs, and Home Health Aides as directed by the Clinical Manager.

8. Maintains accurate and timely documentation of care/services provided.

9. Notifies the primary care provider, as applicable, when there are changes in the patient’s plan of care or clinical changes.

10. Communicates with other team members, as appropriate, regarding the patient’s plan of care, and updates team members when there are changes.

11. Attends and participates in staff meetings, educational in-services, and committee meetings, as appropriate.

12. Participates in the on-call rotation to ensure the agency provides 24-hour nursing services.

13. Reports adverse events, incidents, and grievances/complaints (patients and/or personnel) to the Clinical Manager.

14. Provides education and information to the patient and/or family members as appropriate to their plan of care.

15. Evaluates the patient’s progress and effectiveness in meeting goals and outcomes.

16. Participates in and coordinates the patient’s discharge from service.

17. Participates in the agency’s Quality Assessment and Performance Improvement (QAPI) program, as appropriate.

18. Adheres to the agency’s policies and procedures.

19. Maintains confidentiality of all aspects of job performance.

20. Practices proper infection control and utilizes PPE as indicated.

21. Only performs treatments or special procedures as authorized by the Home Health Agency and deemed competent to perform by the Clinical Manager.

22. Complies with all applicable federal, state, and local laws and regulations in performance of job functions.

23. Performs other related functions as directed.

2. The employee is required to use hands to operate vehicles and office equipment.

Job Types: Full-time, Part-time, Contract, Per diem, PRN

Pay: $55.00 - $120.00 per hour
