We are currently seeking a Care Manager. This position will require travel between Carrie Ellingson Gietner Home and Grand Manor.
Job Responsibilities and Duties:
Conduct comprehensive assessments and evaluations of patient care needs, ensuring compliance with guidelines.
Develop and implement individualized care plans in collaboration with interdisciplinary teams.
Perform utilization reviews to ensure appropriate resource allocation and adherence to Medicare regulations.
Maintain accurate medical documentation and records in EHR systems, ensuring HIPAA compliance.
Provide guidance and support to nursing staff regarding patient care and documentation practices.
Requirements:
Unencumbered RN or LPN license
2–5 years of experience in long-term care
Knowledge of Medicare regulations
Commitment to Total Quality Management
Working knowledge of Case Mix Index, Quality Measures tracking, QRP, and Value Based Purchasing
