Job Summary

We are seeking a compassionate and dedicated Home Care Aide to provide exceptional support and care to individuals in their homes or community settings. Duties

• Assist clients with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting to promote personal hygiene and comfort.
• Support clients with mobility and transfers using appropriate techniques and equipment like Hoyer lifts or patient transfer methods.
• Monitor patient health by taking vital signs, observing changes in condition, and reporting findings accurately to healthcare professionals.
• Prepare nutritious meals tailored to individual dietary needs while maintaining cleanliness in the kitchen area.
• Provide medication reminders and assist with medication administration following HIPAA guidelines and care plans.
• Help clients with light housekeeping tasks including cleaning, laundry, and maintaining a safe environment.
• Offer companionship and emotional support, engaging clients in meaningful conversations or activities to promote mental well-being.
• Assist with transportation for medical appointments or community outings using personal transportation assistance.
• Follow all safety protocols including first aid, CPR certification, manual handling procedures, and emergency response protocols such as hospice or dementia care situations.
• Document care provided accurately in compliance with HIPAA regulations and agency policies.

Experience

Candidates should have prior experience working in home health, assisted living facilities, nursing homes, or group homes. Experience working with individuals with disabilities—including developmental disabilities, neurodevelopmental disorders like autism or Alzheimer's disease—is highly valued. Familiarity with care plans, patient monitoring, medication administration, patient transfer methods such as Hoyer lifts, and managing residents’ (personal needs) is essential. Additional experience in geriatrics, hospice care, memory care, or long-term care environments will strengthen your application. Candidates must demonstrate excellent client communication skills and a strong understanding of safety procedures including first aid and CPR certification. Prior exposure to manual handling techniques, heavy lifting when necessary, and supporting patients with complex health conditions is preferred.
