About The Opportunity
We're looking for a Patient Navigator Intern to support our Health-Related Social Needs (HRSN) care navigation program. Working alongside clinical and operations teams, you'll help patients overcome real-world barriers - housing instability, food insecurity, transportation gaps - that stand between them and consistent, quality care. This role functions similarly to a case manager and is an ideal fit for someone pursuing a career in community mental health, social work, public health, or healthcare operations.
What You’ll Do
A current undergraduate or graduate student, or recent graduate, in social work, psychology, public health, healthcare administration, community health, or a related field. You bring:
Many patients struggle not because of a lack of clinical care, but because of unmet social needs. In this role, you'll directly support access to care, treatment continuity, and long-term health outcomes for some of the most underserved patients in New York City.

What You’ll Do
- Screen patients for health-related social needs: Conduct structured assessments using established screening tools and internal workflows.
- Connect patients to community resources: Help individuals navigate housing programs, food access, transportation support, and other essential services.
- Provide ongoing navigation support: Follow up with patients to ensure continuity and address emerging barriers as they arise.
- Collaborate across teams: Partner with clinical and operations staff to support individual care plans and document progress accurately.
- Support community outreach: Participate in engagement efforts to reach patients who may not yet be connected to services.
- Care coordination: Hands-on case management experience with Medicaid populations.
- Social determinants of health: How unmet social needs directly shape clinical outcomes.
- NYC public systems: Familiarity with community resources, public programs, and local support networks.
- Behavioral health operations: Working within an integrated care model using remote tools and documentation workflows.
A current undergraduate or graduate student, or recent graduate, in social work, psychology, public health, healthcare administration, community health, or a related field. You bring:
- Genuine interest in community mental health, care navigation, or healthcare support
- Comfort discussing sensitive topics with empathy and professionalism
- Strong communication, interpersonal, and organizational skills
- Ability to work independently in a remote environment and stay accountable
- Openness to feedback, supervision, and mentorship
- Willingness to learn new systems and remote workflows quickly
- Fully remote internship
- One required in-person training in New York City (approximately 3 hours)
- Full-time, 40 hours per week for the duration of the summer internship
- Training provided on screening tools, navigation workflows, documentation processes, and remote systems
- Supervision and mentorship from experienced team members
