What are the responsibilities and job description for the Team Lead position at Rehabilitation Support Services, Inc.?
The Saratoga ACT team (Assertive Community Treatment team) is a multidisciplinary team including clinicians, nurses, prescriber(s), and case managers. The ACT team provides intensive, community-based mental health support to people with severe and persistent mental illness. Services are delivered in the person’s home or community, available 24/7, and focus on helping individuals stay stable, avoid hospitalization, and build skills for independent living.
The ACT Team Leader will be responsible initially for the successful implementation of the program model and then managing the day to day operations of the ACT Team. Duties will include (but not be limited to) coordination of individual appointments, collateral contacts, referrals and follow up, intake, discharge planning, safety planning and “double teaming” with other team members and providing coverage for times other staff is off, sick, etc., and regular visits with consumers.
The Team Leader will be responsible for the development and maintenance of collaborative relationships with community partners. The Team Leader role will require significant multi-taking in a fast paced, challenging environment. For the right candidate it will present the opportunity to enhance their management and supervisor experience and demonstrate their skills and experience in a high profile program.
The role will require field-based work, periodic on call coverage, and a willingness to work flexible hours. This is an exciting opportunity for a leader who is looking to transform community healthcare making long lasting positive changes in the lives of complex individuals in the community.
Minimum Qualifications:
- Master’s degree or higher in Social Work, Mental Health Counseling, Public Administration, Nursing, Public Health, Public Policy or a related field.
- Minimum of 3 to 5 years post-masters work experience in the behavioral health and criminal justice field with progressively increasing responsibilities, preferably with the target population.
- Previous management and supervisory experience preferred.
