Rehabilitation Support Services, Inc. is seeking a full time Care Manager for Health Homes Serving Children. The care manager will work in our Otsego County office to provide care management services to youth and families in Otsego, Delaware, Schoharie.

The primary role of a Care Manager is to provide Health Home services to a caseload with the goal of reducing the rate of hospitalizations through engagement in community-based resources, behavioral and physical health care. The Care Manager will work closely with community providers to ensure appropriate delivery of services and conduct assessments to assist in identifying client needs and create individualized plans of care to reflect these needs. Care Managers must be extremely organized, able to multitask/prioritize and work independently.
Requirements:
Bachelor of Arts or Science with two years of experience providing services to youth and families
Bilingual (Spanish) preferred OR Associates Degree with 3 years working with youth preferred
Excellent organizational, writing and computer skills
CANS-NY certification a plus
Other Requirements:
Valid NYS driver's license required
Successful completion of Justice Center and New York State Office of Children and Families Statewide Central Register Database clearance required
