Duties
- Assess patients’ physical conditions through comprehensive evaluations, including anatomy and physiology knowledge, to inform treatment strategies
- Develop and implement individualized care plans targeting specific rehabilitation goals for each patient
- Practice in various settings such as outpatient clinics, hospitals, inpatient units, home health, and community care environments
- Collaborate closely with physicians, nurses, occupational therapists, and other healthcare professionals to coordinate effective treatment approaches
- Manage electronic health records (EHR) and electronic medical records (EMR) systems accurately to document patient progress and treatment details according to clinical standards
- Conduct discharge planning to ensure seamless transition of care from hospital or inpatient settings to home or community environments
- Utilize specialized techniques such as trigger point therapy and physiotherapy modalities to optimize patient outcomes
- Work with diverse populations including pediatrics, geriatrics, neurology patients, and those recovering from orthopedic surgeries or trauma
Qualifications
- Valid license or certification practicing in a physical therapy role within your jurisdiction
- Proven experience in acute care, outpatient clinics, inpatient hospital settings, or outpatient rehabilitation centers
- Strong knowledge of medical terminology, anatomy, physiology, and clinical documentation standards
- Experience working with geriatric patients and familiarity with home health and community care practices
- Ability to develop effective treatment plans tailored to individual patient needs across various specialties such as orthopedics and neurology
- Excellent client communication skills to educate patients about their rehabilitation process and foster motivation
- Proficiency in EMR/EHR systems for accurate documentation of patient care activities
- Knowledge of discharge planning procedures and collaboration with healthcare teams for comprehensive patient management
