What are the responsibilities and job description for the Social Worker - Part Time position at Rego Park Health Care?
We are looking for an indivual who can take on the roll as a dir of social work at our SNF. This job includes but is not limited to,-setting up care plan meetings-working with other departments to coordinate best plan of action for each resident-strong follow up, and people skills-evaluting residents using the MDS assessment-working well with assited livings as well as home care agencys to insure a safe and timely discharge for each indivual resident.
Salary : $72,000 - $110,000
