What are the responsibilities and job description for the Full time Social Worker position at Rego Park Health Care?
We are looking for an individual who can take on the role of Social Worker at our facility. This job includes but is not limited to,
- setting up care plan meetings and documenting
- working with other departments to coordinate best plan of action for each resident
- strong follow up, advocacy and "people" skills
- evaluting residents using the MDS assessment, completing all required care plan and progress note documentation in the EMR
- education and completion of advance directives
- working well with assisted living facilities and adult homes as well as home care agencies to insure a safe and timely discharge for each indivual resident, referring residents to appropriate state run programs.
Nursing Home experience is REQUIRED.
