Regional Hospice is looking for a FULL TIME RN Case Manager for our Home Hospice patients.
We are looking for full time 40 hours, day shift. We offer a generous benefits package including: Vision, Medical, Life, Dental, 401K Match, Vacation/Sick Time as well as the use of a vehicle for work and personal use.
The Case Manager is responsible for the coordination and management of all patient care and other services for patients and their families. The Case Manager works with interdisciplinary team and the patient’s primary care physician to provide quality pain and symptom management, as well as support to the patient and family.
Qualifications
- Registered Nurse with a current license in the State of Connecticut REQUIRED.
- Two (2) years experience as a staff nurse in a hospital, home health care agency, or hospice REQUIRED.
- Advanced credentials in hospice care preferred.
- Transportation required, including proof of auto insurance and a clear driving history.
- Excellent verbal, written and computer skills – ability to communicate effectively with patients, families, physicians, interdisciplinary team members and others.
- Bachelor of Science in Nursing degree preferred.
The RN Case Manager must possess good communication and organizational skills and be able to work in a variety of environments. The RN Case Manager presents a professional attitude and appearance at all times while maintaining strict confidentiality of patient and agency information.
