Regional Hospice is expanding services, and we are excited to recruit a FULL TIME Pediatric Hospice Case Manager & Program Development Coordinator for our pediatric hospice and palliative care patients!  Provide compassionate, developmentally appropriate hospice and palliative care to pediatric patients and their families, while also supporting the expansion and growth of the agency's pediatric hospice program.  In addition to direct patient care, this role collaborates with leadership to build and maintain relationships with health system partners, physicians, and community referral sources to increase awareness, education, and appropriate referrals for pediatric hospice services.    Essential duties include:

Clinical Care: providing skilled nursing care to pediatric hospice patients across the continuum of illness, including symptom management, pain control, and psychosocial support; performing comprehensive assessments of pediatric patients, incorporating physical, emotional, developmental, cultural, and family-centered needs; collaborating with the interdisciplinary hospice team (physicians, social workers, chaplains, child life specialists, and others) to develop and implement individualized plans of care; educating and supporting families and caregivers on disease progression, symptom management, medication administration, and end-of-life care; providing emotional support, anticipatory guidance, and bereavement-sensitive care to patients and families; participating in on-call rotation as required.

Pediatric Program Development & Expansion: partnering with leadership to support the implementation and growth of the agency's pediatric hospice and palliative care program; serving as a clinical resource and ambassador for pediatric hospice services within the community; assisting in identifying gaps, opportunities, and best practices to enhance pediatric service delivery; contributing to the development of pediatric-specific policies, procedures, educational materials, and care pathways as needed; and, supporting quality improvement initiatives related to pediatric hospice care.

Relationship Building & Referral Development: building and maintaining professional relationships with pediatrics, hospital systems, NICUs/PICUs, specialty clinics, case managers, school-based providers, and community agencies; providing education and outreach to referral sources regarding pediatric hospice eligibility, services, and benefits; collaborating with marketing, philanthropy, and leadership teams to support awareness and appropriate utilization of pediatric hospice services\; participating in community events, in-services, and  professional meetings as a representative of the pediatric hospice program; and, maintaining timely and professional communication with referral partners to ensure continuity of care and strong collaboration. 

Documentation & Compliance: Completing accurate, timely clinical documentation in accordance with hospice regulations, agency policy, and payer requirements; ensuring compliance with CMS Hospice Conditions of Participation, state regulations, and accreditation standards; and, participating in interdisciplinary team meetings, care conferences, and quality review activities. 

 

Qualifications:

Required:

  • Current, unrestricted Registered Nurse (RN) license in the state of CT
  • Bachelor of Science in Nursing degree preferred.
  • Minimum of 2-3 years of pediatric nursing experience (acute care, home care, hospice, palliative care, or related specialty) 
  • Valid driver's license with reliable transportation and clean driving record with proof of auto insurance.
  • Ability to work independently in home and community-based settings 

Preferred

  • Pediatric hospice and/or pediatric palliative care experience
  • Certification in Hospice and Palliative Nursing (CHPN), or Pediatric Palliative Nursing (CHPPN), or willingness to obtain
  • Experience with program development, outreach, or referral relationship management
  • Strong presentation and education skills
