LTSS-Facilitator
Location: Hybrid Remote – Saint Joseph, MI
Status: Full-Time | Exempt

At Region IV Area Agency on Aging, we are committed to helping older adults live with dignity, independence, and respect. We are seeking an organized and detail-oriented LTSS-Facilitator to join our growing team.
What You'll Do
- Facilitate cross-functional coordination with internal teams and external partners to arrange comprehensive long-term services and support, ensuring all participant needs are addressed.
- Provide follow-up and monitoring of service arrangement and delivery.
- Help identify and resolve barriers to access and optimal utilization of long-term services and support available to participant.
- Telephone and/or visit participants. Assists in gathering additional information and documentation as needed.
- Provide community outreach and scheduling assistance as needed
- Contribute to maintenance of active, up-to-date person-centered service plans and emergency back-up plans.
- Documents all activity in appropriate software system(s).
- Assists as needed with other projects and assignments that optimize program implementation and delivery.
- Other duties as assigned.
Qualifications
- High school diploma or GED required.
- One year of related experience is preferred.
- Long-Term Care Medicaid knowledge preferred.
- Ability to manage multiple priorities and maintain confidentiality.
- Strong organizational, communication, and customer service skills.

- Valid driver's license required.

Pay: $18.00 - $20.00 per hour
