Job Summary
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a pivotal role in supporting individuals and families facing various challenges, including behavioral health issues, addiction, and social welfare needs. The Case Manager will work closely with clients to assess their needs, develop personalized care plans, and coordinate services to promote their well-being and stability.
Responsibilities
- Conduct comprehensive assessments of clients' needs and circumstances to inform case planning.
- Develop, implement, and monitor individualized service plans that address clients' goals.
- Provide crisis intervention and management support to clients in urgent situations.
- Offer addiction counseling and support for individuals struggling with substance use disorders.
- Collaborate with healthcare providers, social services, and community resources to facilitate access to necessary services.
- Maintain accurate documentation of client interactions, progress notes, and case files in compliance with organizational policies.
- Supervise and mentor junior staff or interns as needed, fostering a supportive learning environment.
- Advocate for clients’ rights and assist them in navigating systems such as Section 8 housing and child welfare services.
- Engage in workforce development initiatives to help clients gain employment skills and opportunities.
Skills
- Proficiency in crisis intervention techniques and crisis management strategies.
- Knowledge of behavioral health principles and practices.
- Experience in social work or related fields with a focus on client advocacy.
- Familiarity with motivational interviewing techniques to encourage client engagement.
- Supervising experience is preferred for overseeing junior staff or interns.
- Understanding of childcare practices and child welfare regulations.
- Knowledge of Medicare processes and resources available for clients.
- Strong organizational skills with the ability to manage multiple cases effectively.
- Excellent communication skills for building rapport with clients from diverse backgrounds.
