The ideal candidate will have a strong background in healthcare admissions, excellent communication skills, and a proven ability to build positive relationships with hospitals, referral sources, residents, and families.
Responsibilities:
Coordinate all aspects of the admissions process from initial inquiry to resident move-in.
Communicate effectively with hospitals, referral sources, residents, and families to facilitate a smooth transition of care.
Maintain accurate census and referral data, ensuring timely and compliant documentation.
Conduct facility tours and provide information about services and programs.
Collaborate with clinical and administrative teams to ensure admissions decisions align with resident needs and facility capabilities.
Develop and maintain strong referral relationships within the community and healthcare network.
Assist in marketing and outreach initiatives to promote the facility’s programs and services.
Qualifications:
Previous experience in healthcare admissions, preferably in a skilled nursing or rehabilitation setting.
Strong understanding of Medicare, Medicaid, and insurance eligibility requirements.
