We’re currently seeking an experienced, detail-driven MDS Coordinator to join our clinical leadership team for a 32-hour per week position. - Coordinate and complete accurate MDS assessments in compliance with state and federal guidelines
- Ensure timely submissions and support care planning processes
- Collaborate with nursing, therapy, and interdisciplinary teams
- Monitor documentation for accuracy, completeness, and compliance
- Support audits, surveys, and quality improvement initiatives
- Active RN or LPN license in good standing
- Previous MDS experience in a long-term care setting preferred
- Strong knowledge of RAI process and Medicare/Medicaid regulations
- Excellent organizational skills and attention to detail
