RegalCare at Glenridge is a skilled nursing and rehabilitation center committed to high-quality, resident-centered care and strong clinical outcomes. We are currently seeking an experienced and detail-oriented MDS Coordinator to join our team in a 40 per hour position. This role is ideal for a clinician looking for a balanced schedule while continuing to play a key role in care coordination and compliance.

What You’ll Do:

  • Coordinate and complete accurate, timely MDS assessments in accordance with state and federal regulations
  • Manage the RAI process and support interdisciplinary care planning
  • Ensure documentation supports quality outcomes and reimbursement
  • Collaborate with nursing, therapy, and administrative teams
  • Assist with audits, surveys, and quality improvement initiatives


What We’re Looking For:

  • MUST have Active RN or LPN license in good standing
  • Prior MDS experience in a long-term care setting preferred
  • IN-House Position
  • Strong knowledge of Medicare/Medicaid guidelines and RAI requirements
  • Excellent organizational skills and attention to detail
  • Ability to work independently and manage deadlines
