Responsibilities:
- Conduct initial and ongoing assessments of patients' and residents' physical, emotional, and social needs
- Develop and implement personalized care plans to address their needs and goals
- Collaborate with interdisciplinary teams to ensure comprehensive care delivery
- Provide counseling and support to patients and residents, as well as their families and caregivers
- Facilitate communication among patients, families, and healthcare providers
- Identify and address social determinants of health that impact patients' and residents' well-being
- Licensed Clinical Social Worker (LCSW) in the state of Massachusetts
- Previous experience in Long-Term Care (LTC) is preferred
- Strong communication, interpersonal, and problem-solving skills
- Ability to work independently and as part of a team
- Compassionate and empathetic approach to patient care
