Responsibilities:
Coordinate all aspects of social services in the facility.
Serve as a liaison between residents, families, and the care team.
Conduct psychosocial assessments and create care plans.
Assist residents and families with discharge planning and referrals to community resources.
Ensure compliance with federal, state, and local regulations.
Maintain accurate documentation and resident records.
Provide support and guidance to social work team members (if applicable).
Requirements:
Bachelor’s Degree in Social Work or related field required; MSW preferred.
Minimum 2 years of experience in a long-term care or healthcare setting.
Familiarity with MDS 3.0 and care planning process.
Strong communication, organizational, and interpersonal skills.
Knowledge of federal and state regulations regarding social services in nursing facilities.
