Position Title: Reentry Case Manager
Reports To: Program Director
FLSA Status: Non-Exempt
Job Type/Classification: Temporary, Full-Time (Employment is grant-funded and contingent upon continued program funding and organizational need.)
Pay Rate: $23.08/hr.-$25.82/hr.
Work Location: On site, 186-188 North Water Street, Rochester, NY 14604
Work Schedule: 35 hours per week, Monday-Thursday, 8a.m.-4p.m. and Friday, 8 a.m.-1 p.m.; work schedule includes a 30 minute unpaid meal period Monday-Thursday
Job Summary
The Reentry Case Manager is responsible for providing direct, client-centered case management services to individuals affected by the criminal legal system. The goal of this position is to enhance public safety and reduce recidivism by assisting clients in achieving stability, self-sufficiency, and long-term success in the community.
This position requires strong collaboration skills and the ability to work within a team-based environment where clients may be experiencing crisis, trauma, or complex barriers. The Reentry Case Manager must demonstrate professionalism, flexibility, cultural humility, and adherence to confidentiality and ethical standards.
Essential Duties
Direct Client Services
- Conduct client intakes, assessments, and ongoing needs evaluations.
- Provide ongoing case management, advocacy, and supportive services.
- Assist clients with navigating systems and accessing resources including:
- Public benefits and entitlements
- Housing stabilization and applications
- Employment and vocational resources
- Healthcare and behavioral health linkage
- Identification acquisition
- Transportation planning
- Support clients in understanding and meeting parole, probation, or court-related obligations.
- Provide crisis response support and maintain professional boundaries in high-stress situations.
Collaboration & Team-Based Practice
- Work collaboratively with internal staff, leadership, and community partners.
- Coordinate care with probation, parole, DHS, housing providers, behavioral health agencies, and other stakeholders.
- Participate actively in team meetings, supervision, and case consultations.
- Maintain a professional presence in a fast-paced environment where client needs may be urgent or complex.
Documentation & Compliance
- Maintain accurate, timely, and complete client records in accordance with agency, regulatory, and funder requirements.
- Utilize agency case management systems and Microsoft Office tools effectively.
- Adhere strictly to confidentiality standards, including HIPAA and agency privacy policies.
- Protect client information and ensure ethical handling of sensitive data.
Professional Development
- Attend and complete required training and professional development activities.
- Maintain relevant certifications and remain current with best practices in reentry services, trauma-informed care, and client engagement.
Program Support
- Assist with outreach, program activities, and special initiatives as assigned.
- Perform additional duties as needed to support program operations.
All other duties as assigned.
Required Minimum Qualifications
- A bachelor's degree is required or an associate’s degree and two years of experience.
- Experience working with individuals impacted by the criminal justice system, mental illness, and/or substance use disorders preferred.
- Demonstrated ability to work effectively within a multi-cultural environment.
- Comfortable working as part of a team in an environment where clients may be experiencing crisis or heightened needs.
- Reliable transportation and valid NYS driver’s license.
- Ability to obtain clearance to enter Monroe County Jail is preferred.

- Certifications such as OWDS, MRT, T4C, or CASAC preferred.
Physical Requirements
- Mobility: Ability to move throughout the building as needed
- Communication: Strong verbal and written communication skills to effectively convey information to staff, stakeholders, and clients.
- Manual Dexterity: Ability to use a computer, phone, and other office equipment regularly.
- Vision: Close vision required for reading reports, creating documents, and working on a computer
- Lifting: Occasionally required to lift and/or move items up to 20 lbs.
- Travel: Ability to travel locally, occasionally or frequently, as dictated by program needs.

- Must maintain auto insurance if using personal vehicle.
