Job Title: Case Manager

Reports to: Executive Director
Family Promise of Ontario County (FPOC) serves families in Ontario County, NY who are experiencing housing instability through multiple programs of care that include emergency shelter, eviction prevention, shelter diversion, and stabilization services. Our team works within trauma-informed care and strengths based practices to support families with dignity, respect, and compassion.
The Case Manager supports families in achieving stability through individualized goal planning, housing support, resource coordination, and ongoing case management services. This role partners with families to remove barriers to housing, connect them with community resources, and build pathways toward long term independence. The ideal candidate is compassionate, organized, collaborative, and committed to empowering families through relationship centered, family focused service delivery.
Duties Include:
- Supports families through a welcoming and respectful intake process.
- Assist families with setting and obtaining goals. This may include job applications and interview preparation; budgeting/financial coaching; getting families connected to necessary community resources.
- Help in securing permanent housing by liaising with landlords.
- Works alongside families to prepare for housing, including support with financial barriers, utilities, and essential household needs.
- Work with the Case Management team to deliver resources, referrals, and workshops as needed related to employment, financial literacy and budgeting, housing plans, and educational planning.
- Develop relationships with partner organizations and maintain a database of resources for families.
- Serve as an advocate for families with community service agencies.
- Provide crisis support to families as needed.
- Conduct weekly individual meetings with families (minimum).
- Document all case management activities accurately within 48 hours.
- On call, provide coverage for nights/weekends (approximately 1week/month).
- Keep case file records for every family, meeting program, funder, and family needs.
- Must have a valid driver’s license and willingness to use a personal vehicle to transport guests if necessary (reimbursed for mileage).
- Some lifting of materials/donations and helping moving families into their own apartments.
- Complete other duties as assigned.
Skills and Qualifications:
- The successful candidate is enthusiastic and can display good judgment, take initiative, and help families achieve outcomes. Must have a can-do, figure-it-out attitude.
- Four-year degree (BSW, psychology, or related degree) preferred.
- Minimum 3 years’ experience in direct service related to employment placement, housing/shelter, and/or family services preferred.
- Expected case management skills include trauma-informed care, housing-first and a strength-based approach.
Agency Expectations:
- Complies with agency standards of conduct and professionalism.
- Demonstrates knowledge of the program, mission and services of FPOC.
- Complies with all policies and procedures in Employee Handbooks.
- Is punctual and consistent in reporting for scheduled work shifts to assure coverage.
- Maintains client confidentiality.
- Is able to work in an alcohol and drug free environment.
- A 90-day probationary period applies to all new hires.
Compensation:
- Monthly stipend for health insurance.
- Generous PTO.
- $48,000 - $53,000 depending on experience.
- Mileage reimbursement for work-related car travel.
Hours:
- Four days a week, 10 hour shifts
