Waiver Case Manager I

Redeemer Services
Saint Paul, MN Full Time
POSTED ON 1/2/2026
AVAILABLE BEFORE 3/4/2026

Role Summary

As a Case Manager, you are a dedicated advocate for individuals with physical and mental disabilities. Your mission is to empower clients to lead fulfilling, self-directed lives. You will work collaboratively with individuals to develop personalized support plans, navigate complex service systems, and ensure they have the resources and autonomy to make informed decisions about their own futures.

Key Responsibilities

  • Individualized Planning: Partner with clients to assess their unique needs and goals, creating comprehensive, person-centered support plans.
  • Service Coordination: Identify and refer clients to appropriate disability services, community resources, and healthcare providers.
  • Advocacy & Empowerment: Ensure clients are the primary decision-makers in their own lives, providing them with the tools and information necessary for self-advocacy.
  • Case Management & Monitoring: Regularly monitor the delivery and effectiveness of services, adjusting support plans as needs evolve to ensure the highest quality of care.
  • Resource Navigation: Guide individuals through the complexities of state and local benefit programs to secure essential funding and support.

This role is likely a great fit if:

  • You are a Natural Advocate: You feel a strong drive to ensure people's voices are heard and their rights are respected.
  • You Value Autonomy: You believe that every individual, regardless of ability, should be responsible for their own decisions.
  • You are a Problem Solver: You enjoy navigating systems.
  • You are Highly Organized: You will need to manage multiple cases simultaneously without letting important details or deadlines slip through the cracks.
  • You Practice Active Listening: You listen more than you speak, ensuring you truly understand a client's goals before making recommendations.

This role might NOT be the best fit if:

  • You prefer a predictable "desk job": This role often involves changing priorities, urgent client needs, and navigating different service environments.
  • You prefer making decisions for people: Our model is person-centered; we guide and support, but we do not dictate how our clients should live.
  • You struggle with paperwork: While the heart of the job is people, a critical part of the job is creating plans and paperwork for service approval.

Qualifications

  • Education: Accredited four-year degree in Social Work, Special Education, Psychology, Sociology, Nursing, Human Services, or a related field.
  • OR a four-year degree in any field PLUS one year of experience as a Social Worker/Case Manager/Care Coordinator.
  • Skills: Effective communication, strong time management, and intermediate computer skills for documentation.
  • Requirements: Must possess a valid Driver's License and car insurance for client visits in the Twin Cities metro; must pass a DHS background check.
  • Preferred: Experience with Mental Health Targeted Case Management (MH-TCM), Relocation Service Coordination (RSC), or group-home setting, Housing Stabilization Services.
