Role Summary
As a Case Manager, you are a dedicated advocate for individuals with physical and mental disabilities. Your mission is to empower clients to lead fulfilling, self-directed lives. You will work collaboratively with individuals to develop personalized support plans, navigate complex service systems, and ensure they have the resources and autonomy to make informed decisions about their own futures.
Key Responsibilities
- Individualized Planning: Partner with clients to assess their unique needs and goals, creating comprehensive, person-centered support plans.
- Service Coordination: Identify and refer clients to appropriate disability services, community resources, and healthcare providers.
- Advocacy & Empowerment: Ensure clients are the primary decision-makers in their own lives, providing them with the tools and information necessary for self-advocacy.
- Case Management & Monitoring: Regularly monitor the delivery and effectiveness of services, adjusting support plans as needs evolve to ensure the highest quality of care.
- Resource Navigation: Guide individuals through the complexities of state and local benefit programs to secure essential funding and support.

Qualifications
- Education: Accredited four-year degree in Social Work, Special Education, Psychology, Sociology, Nursing, Human Services, or a related field.
- OR a four-year degree in any field PLUS one year of experience as a Social Worker/Case Manager/Care Coordinator.
- Skills: Effective communication, strong time management, and intermediate computer skills for documentation.
- Requirements: Must possess a valid Driver's License and car insurance for client visits in the Twin Cities metro; must pass a DHS background check.
- Preferred: Experience with Mental Health Targeted Case Management (MH-TCM), Relocation Service Coordination (RSC), or group-home setting, Housing Stabilization Services.
Case Manager I
- No experience working as a contracted case manager in Minnesota.
Case Manager II
- Minimum of 12 months working as a contracted case manager for an agency located in Minnesota.
Schedule:
- Monday through Friday, 8:30 am to 5 pm. (Flexible start time available).
Work Environment: The Path to Remote Work
We provide a clear, performance-based path to a hybrid or fully remote work schedule:
- Onboarding (Days 1–60): Mandatory in-office work to ensure successful orientation, training, and mentorship.
- Day 60 Milestone: On your 60th day of employment, you will receive your first scheduled work-from-home day.
- Post-Day 60 Transition: Following the first day, subsequent work from home days will be granted based on meeting specific performance criteria.
- Full Flexibility: After the 6-month probationary period, Case Managers may work up to 5 days a week from home and become eligible for compressed work schedule (4x10) based on sustained performance.
Salary Range:
- Case Manager I: $22 - $25
- Case Manager II: $25 - $29
Sign On Bonus:
- Case Manager I: $1000 paid over a period of 6 months.
- Case Manager II: $1500 paid over a period of 6 months.
