Were seeking a dedicated Social Services Care Coordinator to join our multidisciplinary team. In this role, youll be the connection between individuals served and community resourcesensuring smooth transitions, coordinated care, and compassionate support. Your work will help create stronger pathways to recovery, stability, and wellness.
What Youll Do
- Interview individuals served to gather background information and social history
- Develop and implement discharge plans, arranging aftercare services and community linkages
- Provide case management and connect individuals served with benefits, facility programs, and community resources
- Facilitate therapeutic activities as assigned
- Collaborate with the treatment team in the development and execution of treatment plans
- Communicate with internal and external partners to support care transitions
- Document services and progress in a timely and accurate manner, meeting legal, contractual, and regulatory standards
Education
- Bachelors degree in social work, counseling, psychology, or a related field (required)
- Masters degree preferred (may be required by state or contract)
Experience
- Relevant professional experience and/or training with the identified population (required)
Licenses/Certifications
- Licensure appropriate to discipline (preferred)
