Case Manager – Virtual Intensive Outpatient Program (IOP)

Position Summary

The Virtual IOP Case Manager provides care management services to adults with substance use disorders, serious emotional disturbances, and severe and disabling mental illnesses. Under the supervision of the Program Supervisor, the Case Manager partners with consumers to assess needs, develop individualized treatment plans, coordinate care across service systems, and support recovery-focused goals. This role requires meeting caseload and billing expectations on a monthly, quarterly, and annual basis.

Essential Job Functions

  • Assess and evaluate adults to determine appropriateness and need for care management services
  • Assist consumers with entry into services and monitor progress throughout the continuum of care
  • Develop, coordinate, and monitor Individual Treatment Plans (ITPs) in alignment with agency policy and procedure
  • Ensure treatment plans are completed within 5 encounters or 21 days, whichever is sooner, and that clinical assessments are updated annually
  • Coordinate treatment team meetings and ensure treatment plans are outcome-based and recovery-focused
  • Educate consumers on recovery markers and incorporate them into treatment plan evaluation
  • Screen consumers for Medicaid eligibility and assist with enrollment as applicable
  • Complete required documentation, reports, and applications within established timelines
  • If serving consumers with a mental health diagnosis, complete SDMI designation and Level of Impairment (LOI) prior to initiating case management services; for SUD diagnoses, the SUD diagnosis serves as the criteria for service entry
  • Collaborate effectively with internal team members and external community providers
  • Meet bi-weekly with the Program Supervisor to review consumer cases and service delivery
  • Attend required trainings, workshops, and conferences; share relevant learning with team members as requested
  • Perform other duties as assigned

Virtual Service Delivery

Services are delivered primarily via telehealth through the agency's virtual IOP platform. All telehealth documentation must reflect clinical appropriateness for remote delivery. Case Managers are expected to maintain flexible scheduling to accommodate consumer availability, including evening hours as needed.

Community Based Psychiatric Rehabilitation Services (CBPRS)

Case Managers may also provide CBPRS, documented separately with an individual superbill. CBPRS consists of intensive behavioral management and stabilization services designed to reduce disability, restore functioning, and support social integration. Specific expectations include:

  • Skills training that includes clear description, modeling, practice, and performance feedback
  • Consultation with family members or key support persons as clinically indicated
  • Individual CBPRS up to 2 hours and group CBPRS up to 2 hours per 24-hour period; exceptions require department approval
  • Group CBPRS limited to 8 adults per staff member
  • Face-to-face delivery is preferred; telehealth is permitted when clinically indicated or when access barriers exist, with documentation of rationale and attempts to identify local supports

Attendance and Scheduling

This position requires flexible scheduling to meet consumer needs. Schedule changes will be communicated in writing. Attendance at staff meetings and required trainings is expected and may occasionally require schedule adjustments.

Requirements:


Minimum Qualifications

  • Bachelor's degree in Social Work, Psychology, or a related human services field from an accredited college; or an equivalent combination of education and at least two years of full-time experience serving adults with mental illness, substance use disorders, or a related human services population
  • Individuals who have developed the necessary knowledge and skills through lived experience as a consumer, provider, or advocate of mental health services may be considered; equivalency must be documented per agency policy
  • Reliable internet connection and a private, confidential workspace suitable for telehealth service delivery

Knowledge, Skills, and Abilities

  • Working knowledge of Montana Medicaid behavioral health regulations, SDMI criteria, Level of Impairment (LOI) documentation standards, and targeted case management requirements
  • Ability to assess and respond to the clinical and functional needs of adults with SUD and/or serious mental illness
  • Familiarity with community resources and the ability to connect consumers with medical, social, vocational, recreational, or other support services
  • Effective verbal and written communication skills sufficient to build trusting therapeutic relationships and produce compliant clinical documentation
  • Strong interpersonal skills, empathy, adaptability, and a positive approach to working with diverse populations
  • Ability to work collaboratively within a multidisciplinary treatment team and with external community providers
