Job Title: Home Care Occupational Therapist
Job Summary:
We are seeking a compassionate and dedicated Home Care Occupational Therapist to provide rehabilitation services to patients in the comfort of their own homes. The Occupational Therapist will assess and treat patients who have difficulty performing daily activities due to physical, mental, or developmental disabilities, helping them regain independence and improve their quality of life.
Responsibilities and Duties:
- Conduct in-home assessments to evaluate patients' functional abilities and needs
- Develop personalized treatment plans to address patients' goals and needs
- Provide therapeutic interventions to improve patients' strength, endurance, coordination, and mobility
- Teach patients and their caregivers techniques to enhance independence in daily activities
- Monitor patients' progress and adjust treatment plans as needed
- Document patient evaluations, progress, and discharge summaries in a timely manner
- Collaborate with other healthcare professionals to ensure comprehensive care for patients
Qualifications and Skills:
- Bachelor's or Master's degree in Occupational Therapy
- Valid state license to practice as an Occupational Therapist
- Prior experience working in home care or rehabilitation settings preferred
- Strong communication and interpersonal skills
- Ability to work independently and manage a caseload effectively
- Knowledge of current trends and best practices in Occupational Therapy
