Purpose of this position The purpose of this position is to provide direct care to residents, under the supervision of a licensed nurse, in accordance with facility policies and procedures. Qualifications · State-required training/certification. · Active registration on state Nurse Aide Registry. Delegation of authority Authority is delegated to the individual in this position to: · Provide care to assist residents with their activities of daily living as well as other activities designated on the resident's plan of care or as assigned. · Report resident needs and concerns to a licensed nurse. 1. Follow written and oral directions. |

|
2. Follow established performance standards and perform duties according to nursing service policies and procedures. |
||
|
3. Assist new employees in following established facility policies and procedures. |
||
|
4. Identify resident problems and concerns and report them immediately to a Charge Nurse or a licensed nurse. |
||
|
5. Complete assignments timely, completely and accurately. |
||
|
6. Conduct resident rounds as assigned. |
||
|
7. Has successfully completed an approved CNA program as required by state and/or federal regulations. |
||
|
8. Identify safety hazards and emergency situations and initiate corrective action. |
||
|
9. Participate in in-service classes as assigned and complete assignments. |
||
|
10. Assume personal responsibility for following facility procedures related to control of equipment and supplies within the unit. |
||
|
11. Report incidents and/or unusual problems according to established facility procedures. Report within shift it occurs. |
||
|
12. Participate in interdisciplinary team and other facility meetings as assigned. |
||
|
13. Participate in the development of an individualized plan of care of assigned residents. |
||
|
14. Review care plans and provide care as outlined. |
||
|
15. Record the care and treatment provided to the resident and the resident's response or lack of response to care provided. |
||
|
16. Maintain confidentiality of resident and facility information. |
||
|
17. Report changes in resident conditions to Charge Nurse or Nurse Supervisor. |
||
|
18. Follow facility procedures for admission, discharge and transfer of residents. |
||
|
19. Complete documentation accurately, timely, following facility policies and procedures. |
||
|
20. Listen to resident and family complaints and report to Charge Nurse. |
||
|
21. Ensure residents have call lights in reach. Answer call lights promptly. |
||
|
22. Assist with activities of daily living as designated in each resident's care plan. |
||
|
23. Participate in the planning and implementation of the individualized plan of care for assigned residents. |
||
|
24. Ensure residents are given adequate assistance with meals, nourishment and transportation to meal service area(s). |
||
|
25. Check name bands on all assigned residents daily and report to Charge Nurse any bands that have been removed or need to be changed. |
||
|
26. Consistently work cooperatively with residents, charge nurses, treatment nurses, restorative nurses, other nursing assistants, physicians, families, consultant personnel and ancillary service providers. |
||
|
27. Observe all facility safety policies and procedures. |
||
|

29. Report for work in a clean, neat uniform and consistently present an appropriate professional appearance. |

||

36. Be responsible for the safety of residents under his/her supervision. |
||

37. Accept assigned duties in a cooperative manner. |
||
|
38. Complete annual 12 hours in-service programs as designated by federal regulations. |
||
|
39. Attend training in dementia management and resident abuse prevention as assigned. |
||
|
40. Participate in all compliance and ethics training programs. |
||
|
41. Use only authorized abbreviations established by the facility when recording information. |
||
|
42. Agree not to disclose assigned user ID and password for accessing resident/facility information. Promptly report suspected or known violations of such disclosure to the Charge Nurse or Compliance and Ethics Officer. |
||
|
43. Agree not to disclose resident's protected health information and promptly report suspected or known violations of such disclosure to the Charge Nurse or Compliance and Ethics Officer. |
||
|
44. Assist residents with bathing, oral care, dressing/undressing, toileting, perineal care, catheter care, grooming, eating and taking/recording vital signs in accordance with facility policies. |
||
|
45. Strip bedding, change linens and make resident beds as assigned. |
||
|
46. Ensure that department computer workstations left unattended are properly logged off. |
||
|
47. Report all accidents/incidents to your supervisor no matter how minor they may be. Report such occurrences on the shift in which they occur. |
||
|
48. Report any known or suspected unauthorized attempt to access facility's information system. |
||
|
49. Report suspected resident abuse immediately to the Administrator. |
||
|
50. Promptly report suspected violations of the facility's compliance and ethics program to the facility's Compliance and Ethics Officer. |
||
