Direct Care Professional
Location: Gallup, NM
Job Type: Full-Time, Part-Time

Your Responsibilities:
As a Direct Care Professional, you’ll support individuals with disabilities in leading fulfilling lives by providing personal care, emotional support, and assistance with daily living tasks.
Key Duties Include:
1. Direct Care:
- Assist clients with daily living activities, such as grooming, bathing, and meal preparation.
- Understand and implement each client’s Individual Support Plan (ISP), therapy plans, meal plans, medical crisis plans, health care plans, goals, and objectives.
2. Documentation:
- Maintain accurate and timely records, including:
- Home records.
- Communication logs.
- Flow sheets and progress notes.
- Medication administration records.
3. Routine Cleaning and Household Maintenance:
- Ensure a clean, safe, and welcoming home environment by performing basic cleaning tasks, assisting with laundry, and preparing meals.
4. Communication:
- Engage meaningfully with clients, coworkers, families, and guardians.
- Collaborate with service providers and team members.
5. Additional Responsibilities:
- Attend required training sessions.
- Perform other job-related duties as assigned by the manager or program director.
Qualifications:
- Education: High school diploma or equivalent (Required).
- License: Valid driver’s license (Preferred).
- Shift Availability:
- Day Shift (Preferred).
- Night Shift (Preferred).
- Overnight Shift (Preferred).
- Relocation: Must reliably commute or plan to relocate to Gallup, NM before starting work.
