Essential Responsibilities : Assess and monitor patients' conditions, managing pain and symptoms effectively.Administer medications, including pain management and comfort care treatments.Develop and implement individualized care plans based on patient needs.Collaborate with physicians, social workers, chaplains, and other healthcare professionals to ensure comprehensive care.Communicate regularly with patients, families, and caregivers to provide education and emotional support.Coordinate and schedule visits, treatments, and follow-ups based on patient needs.Requirements : VT / Multi State RN License- requiredCurrent Basic Life Support issued by the American Heart Association- required
