Starting Salary Range: full-time - $86,575- $94,398 salary Office Benefit Package (On-call expenses): $50/Month subject to change

Schedule: Monday - Friday day hours, 40 hours per week

Join our Registered Nurse - Float team in providing comprehensive, integrated care to adults living with mental illness, substance use disorder, and associated medical conditions.

The Registered Nurse is a fully integrated member of the agency and will float to various programs within RADIAS Health and works with other members of the treatment team to assist clients in obtaining and maintaining optimum quality of life. Nursing services include monitoring, education, and coaching to clients regarding medical, mental, and chemical health.

Duties and Responsibility

  • Have knowledge of the operational expectations and policies and procedures for each assignment area.
  • Conduct nursing assessments.Participate in and conduct parts of the person-centered comprehensive assessment of psychiatric history, mental status, and diagnosis; physical health and dental health; use of drugs or alcohol; and other aspects of the assessment.
  • Take the lead role and participate in providing medication administration, medication education and medical services.
  • Manage and follow systems of getting medication to clients and integrate medication administration tightly into clients' individual treatment plans.
  • Administer injectable medications to clients using best practice
  • Manage pharmaceuticals and medical supplies.
  • Coordinate, schedule, and administer medical assessments of client physical health, make appropriate referrals to community physicians, and coordinate psychiatric treatment with medical treatment.
  • Maintain communication with other care providers or concerned parties involved in client's care.
  • Update client's medical profile as needed
  • Assess the health needs of clients served and facilitate the development of treatment and crisis plans to address identified need areas: Immediate health needs, physical and mental health status, health history, social, economic, and cultural impact on health needs, chemical use, health habits and ADL's, support and resources available, medication regimen, ability to do self-meds, strengths, knowledge base and motivation for change.
  • Assist clients with managing their medications, including obtaining necessary prescriptions, medication administration, and monitoring side effects. Instruct clients in medication self-management as needed.
  • Share resources and consult with other agency employees as they work with clients, including making referrals and recommendations for care.
  • Educate the team in area of bio/psycho/social nursing.
  • Provide informal and formal training to clients and team members to enable clients to perform self-care activities.
  • Support the medication administration certification process for RSS Mental Health Counselors, IRTS staff, and any other appropriate staff as directed.
  • Take and document vitals on clients as appropriate.
  • Process all prior authorizations in collaboration with prescriber
  • Keep appropriate records and do necessary charting as required.
  • Supervise all aspects of medication administration.
  • Treat medical problems requiring nursing judgments and coordinate appropriate follow-up.
  • Transcribe all physicians' orders on the Physician Order Sheets.
  • Responsible for medication destruction (with Pharmacist for narcotics).
  • Coordinate OSHA regulation requirements.
  • Provide courteous and respectful care to all clients.
  • Work effectively and professionally with other staff members.
  • Complete TB Gold protocol tests for new hires and agency employees as needed.
  • Review all medication errors and narcotic off counts as relevant to program requirements.
  • Arrange for biohazard disposal, medication audits and lab tests if needed.
  • Provide education or refer clients to educational resources for optimal health.
  • File reports with Vulnerable Adult, the Ombudsman's Office, Child Protection, and other mandated/required reports when appropriate.
  • Act as a care coordinator for assigned clients.
  • The Manager of the program to which you are assigned will determine the duties when assigned to the program.
  • Participate in evening, weekend and holiday coverage as needed, unless already approved for time off.
  • Be available for on-call hours specific to program needs.
  • Will be available for after-hours calls from RSS staff every three weeks at minimum; the purpose of the communication with staff is to provide direction and clarify issues related to medication administration.
  • Must be able to transport clients in the community.
Requirements
  • Current license as a Registered Nurse in the State of Minnesota.
  • Preferred: bachelor's degree and experience in inpatient, outpatient, or community psychiatric setting.
