Job Title: Social Work Case Manager
FLSA Status: Exempt
Accountable To: Director of Programs
Summary: Works within the interdisciplinary model providing psychosocial support services to a primary case load and to those receiving primary case management by a nurse case manager. This includes direct clinical practice with patients and client systems, and interaction with community agencies.
Essential Duties & Responsibilities:
- Assess and supervises a primary caseload of 25-40 high acuity patients with co-occurring disorders.
- Conduct psychosocial assessments, develop and implement treatment plans for patients.
- Assist patients in accessing community resources such as Orange County Social Services, Social Security as well as other non-profit social services agencies.
- Provide crisis intervention, short term mental health services and psychosocial case management as needed and appropriate.
- Assist in the development of agency programs to address psychosocial needs of patients.
- Oversee and manage patients on Ryan White and Medi-cal Waiver programs.
- Provide written documentation of work and timely submission of statistics and reports.
- Attend monthly interdisciplinary team and staff meetings.
- Make hospital and home visits for purpose of assessment and provision of services as needed.
- Minimum of 3-5 years experience since graduating with Masters. (Preferred)
- Knowledge and understanding of psychosocial issues of HIV infected individuals, a strong background in resources development and patient advocacy is necessary.
- Experience in medical social work, substance abuse treatment, family therapy and/or working with indigent patients helpful.
- Knowledge of community resources such as Orange County Social Services, Social Security, as well as other private non-profit social service agencies.
