Job Overview
We are seeking a dedicated and experienced Registered Nurse (RN) specializing in Home Health services to join our healthcare team.
Position: Registered Nurse – Home Health
Location: 744 Horizon Ct Ste 128, Grand Junction, CO 81506-3946
Schedule: Full-time, 40 hours/week (M–F, 8am–5pm; weekend on-call required)
License Requirement: Active RN license
Mileage Reimbursement: $0.42/mile
Counties Covered: Mesa and Garfield
Productivity Requirement: Minimum of 30 visits/week
Preferred Skills: HCHB, OASIS, Home Health
Weekly Pay Package
Weekly Tax Free Stipend to be paid $1,243.34
Weekly Straight Pay (Taxable) $1,040.00
Weekly Gross $2,283.34
Role Summary:
The Registered Nurse provides skilled nursing care, health maintenance, disease prevention, and case management for individuals in home and community settings. Responsibilities include assessment, care planning, implementation, evaluation, and supervision of staff to ensure quality patient outcomes.
Key Responsibilities:
· Assess, plan, implement, and evaluate nursing care for individuals with acute, chronic, or wellness needs.
· Collaborate with interdisciplinary teams to coordinate health and safety supports.
· Manage medication processes including ordering, monitoring, discontinuation, and disposal per physician orders and policy.
· Document patient health status, progress notes, and required nursing reports (daily, monthly, quarterly, annual).
· Supervise LPNs/LVNs and provide direction as needed.
· Schedule and follow up on health-related appointments and implement physician recommendations.
· Maintain communication with inpatient nursing and discharge coordinators during hospitalizations.
· Provide new employee orientation and annual recertification training on health and safety topics.
· Train unlicensed staff on individual-specific health, safety, and disease prevention needs.
· Ensure compliance with infection control, equipment, and supply maintenance standards.
